Volunteers and Background Checks

Per Policy 8120, any individual who volunteers to work in the school, on any school-sponsored activity, who works with or has access to students shall submit to a criminal history records check and shall be screened through the Internet site for the Sex Offenders Registry (SOR) list prior to being allowed to participate in any activity or program.

Where to submit the form?

Complete and return the form to the Stone Bank School office. You may also email the completed form to n.brown@stonebank.k12.wi.us, or printed copies are available upon request.
